Radiotherapy as a field has a long tradition of high technology involvement, and practitioners are used to frequent technology advances. Recently, radiotherapy clinics have implemented advanced imaging for planning and treatment verification, rotational intensity modulation radiotherapy delivery as well as intensity modulated proton therapy. The ability to deliver the correct dose timely, efficiently and with a very high geometrical precision is possible at most modern radiotherapy departments. The question is therefore whether we have reached a point when new technology gets introduced and tested but do not offer any benefits for radiotherapy patients. The current presentation and following discussion will take a critical view on the current literature on radiotherapy technology and its clinical evidence.
